Fire service records requests need to be in writing by email below or mailed to AFD Headquarters 18425 Timber Forest Drive, Humble, TX 77346 ATTN Fire Records. There is no cost for a Fire record, except under special circumstances as determined by the Chief of Department.  

EMS/Ambulance records requests need to be in writing to Koronis Revenue Services at 5330 Spring Cypress Rd. Spring TX, 77379 or fax to 281-653-6997. The cost of an EMS/Ambulance record is $40.00. The fee is waived for patients requesting their own records, and for the District Attorney's office. 

In accordance with Federal and State Privacy Laws we may be prohibited from disclosing patient medical records in some cases. To obtain patient medical reports, you must comply with the laws governing protection of these reports (HIPAA). Once you are finished with these reports you must return them, properly store them, or completely destroy them. Records requests without a Records Request form, an incomplete form, or lacking the required attachments cannot be processed.
